For customer data collection you can create a simple form using Google Forms that can be shared via a link so it can be filled out by customers on their own devices or by a volunteer at the door.
It allows you to create your own questions, set some basic validation and export the data in multiple formats. You are also able to deactivate and reactivate the form whenever you choose, so you can switch it off when the clubhouse is closed to the public. It also allows you to delete all responses whenever the creator chooses.
